Home Care Packages Program is one of the ways that older Australians can access affordable in-home support care and services.  The program is designed for those who have complex care needs beyond what the Commonwealth Home Support Program can deliver.

Once you have been assessed as eligible and the level of a Home Care Package, you will be placed in the National Waitlist for a priority queue. When a package is available, you will receive a letter informing you that an assessed level of a package is assigned to you.

What if I need help while I am waiting for my Home Care Package?

If you are advised, you have been approved for a Home Care Package and have been placed on the national waiting list. Your assessed care needs will be classifiedinto two priority categories Medium for level 1 & 2 and High for level 3 & 4.  Home Care Packages Program is regarded highly to support older Australians; this means it may take sometime before your package is assigned.  The waiting time for a package is three to six months.  

There are a number of options for you while your Package is assigned:

Commonwealth Home Support Program (CHSP):

The CHSP services can be put in place while you wait for a Home Care Package and accessing CHSP will be a quick process.

The CHSP provides entry level home care service and often is delivered under an interim measure.  You will be expected to pay towards some of the services, however most of the services will be subsidised by the Government.

Interim Package

You will be advised that a lower level of Package, than you were assessed for, can be made available.  This means you can start receiving services and support rather than wait until your approved package is assigned. The assessor will discuss this option with you while you are being assessed.   If you decide to accept the interim package, you will not lose your approved level of Home Care Package.

Private Home Care Service

One option would be to pay for the private home care service while you are waiting for a Home Care Package. In Australia, there are many private home care providers which are not regulated or receive Government funding.  You need to think about what services you need and whether you can afford private home care services.  The benefit of accessing the private home care service is there is not waiting time and you can get the services straight away.

When your package is assigned, you could continue with your private home care provider under the Consumer Directed Care principles and use it as a “top up” for services that are not funded by your Home Care Package.

Chronic Disease Management Plan (CDMP)

You may want to speak to your doctor as he/she is the best person to discuss your health issues. With your doctor’s support, you may be eligible for assistance while you wait for your Home Care Package.

The CDMP is available to older Australians who have complex health or chronic conditions.

Your can receive five fully-subsided sessions with an allied health professional for a range of ailments.  This could include conditions like asthma, stroke, cancer, diabetes, cardiovascular disease, musculoskeletal conditions and more and your doctor will coordinate the service recommended on the CDMP.

To access the CDMP, you do need to have a chronic condition for over six months.  The program is designed to assist patients who need a structured care approach to recover or manage the condition.

If you have an urgent need because of sudden health changes, you can contact My Aged Care and request your position to be moved to the top of the list. The high priority is given to older Australians who are in more urgent need of a Home Care Package to live at home safely and receive their care they need.

Services funded by the Home Care Packages program

Home Care Package is a Commonwealth Government funded home-based program.  It aims to assist older Australians to stay at home for as long as appropriate.  This Program allows eligible customers to purchase services that are in the list of Inclusive Items.  Below are some inclusions

Personal care such as assistance with showering, dressing and grooming.
Assistance with grocery shopping, meals preparation and helping with eating.
Continence management
Goods, Equipment and Assistive Technology (GEAT) such as a walking frameor mobility scooter
Nursing, allied health and therapy services
Transport and personal assistance for appointments
Management of skin integrity including bandages, dressing, and skin treatments

When you access the allied health services, the professionals must be registered with Australian Health Practitioner Regulation Agency (AHPRA) or self–regulated by another national professional association.

GEAT refers to items that help you to manage a range of health conditions, reduce risk of falls, assist with daily activities that may present a challenge or minimise your reliance to your family.  These items include bendable cutlery, walking sticks / frames, raised toilet seas and those costly supports such as scooters, specialised chairs, wheelchairs and modified beds.  There are three categories for GEAT items:

Prescribed GEAT
Bed transfer rails, walking frames, scooters.

Under advice GEAT
Toilet frames, shower chairs, kitchen stools, magnifiers, personal alarms

Low risk GEAT
Jar openers, long handled shower sponges, long handled dustpan and brush, sock aids, large print calendar

What to do after you have been assessed for a Home Care Package?

After you have been assessed by an Aged Care Assessment Team (ACAT), you will be advised to wait for a letter confirming your eligibility and the level of a Home Care Package.  

During this time, there are steps you can take to get yourself ready before the letter arrives:

Monitor your application status.

To accessing a Home Care Package, you must be assessed by a regional Aged Care Assessment Team, an assessor will contact you within 2-6 weeks to organise an assessment.  You will also receive a My Aged Care welcome pack containing helpful information and inform you what your next steps will be.

To check the status of your Home Care Package application, you can log in to your My Aged Care Online Account or calling 1800 200 422.  After the ACAS assessment is completed, you can track your application status using the online application portal or by calling the above number. 

What is the priority level?

There are two priorities for home care packages – medium and high. The priority indicates how quickly the customer requires the package. The default priority will be medium priority with only a small percentage of customers who are at immediate risk being approved as high priority.

Do ACAT approvals expire?

No, the Home Care Package approval does not expire, unless the ACAT approval was granted only for a specific period.

What if my situation changes since the Assessment?

You may find that since your last assessment, your needs have changed, perhaps some other daily tasks have become more difficult for you, thereforeit is a good idea to contact My Aged Care for a reassessment.

How do you find a home care package provider matches my needs, requirements and preferences?

You should compare prices and the quality of service that they can expect to receive from each provider, as there is a significant difference in cost and the type of service each provider offers. It is also important to consider the organizational culture and level of customer service the customer can expect to experience from a provider.

You should find a Provider who can mange your funds efficiently to cover the services you need both for the current and the future needs.
