Our main objective is to ensure that all older Australians are well-informed about the support options available to them through Home Care Packages, allowing them to reserve their independence in their own homes for as long as possible. Before engaging with a potential customer, we conduct a thorough preliminary assessment to identify their specific care needs, requirements, and preferences.  Our consultant team strictly follows and implements the Partners’ onboarding process and documentation. By completing these tasks, our Partner can have peace of mind knowing that all initial documents have been finalised and the potential customer is ready to sign when we reach out to them.  The consultants responsible for conducting the person-centered assessment, goal-oriented care plan, and consumer-directed budget possess extensive skills and experience in the Home Care Package program.
