So you want to change your Home Care Package provider

If you are unhappy with your current Home Care Package provider, it is advisable to speak to your Provider to sort out the concerns first. If the situation continues, you can commence looking for an alternative Home Care Package provider. Changing tothe Home Care Package provider is easy and at no cost to you.

There are three steps to change your Home Care Package provider:

Inform your current provider in writing that you wish to change provider.  This can be done via email, fax or a letter.  You may need to give them 2-4 weeks’ notice, depending on your Home Care Agreement.  You can also talk to your Provider regarding the notice period and this will assist you to write the letter.
Contact My Aged Care on 1800 200 422.  You will need to let them know that you are changing providers so they can reactivate your Home Care Package referral code.
